Thank you for choosing to refer your patient to SL Endodontics. Your patient will be treated with care and respect, then returned to your practice for their restorative treatment and ongoing care.

Send the referral
Use the form below with the tooth, your findings and the reason for referral. We confirm receipt.

Assessment
We contact your patient and carry out an assessment of the referred tooth or problem.

Agreed treatment
The agreed endodontic treatment is carried out under the operating microscope.

Report & return
You receive a clinical report and your patient returns to you for the definitive restoration and ongoing care.

Refer to SL Endodontics
Referrals are accepted for: root canal treatment and retreatment, apicectomy and endodontic microsurgery, cracked teeth, dental trauma, and diagnosis of tooth pain.
- Send radiographs by secure email once we confirm receipt.
- Mark urgent cases and we will prioritise them.
- Please do not include detailed medical history in the form.
